The project represents a return to the industry where Vitenas began his operating career in 2010. Working inside a Houston specialty medical practice, he became increasingly involved in the systems behind scheduling, patient communication, reporting and day-to-day operations. Those operational problems eventually led him to teach himself SQL and software development.
In 2016, Vitenas began building BIVI, a custom software platform designed around needs he encountered inside the practice. The system connected appointment reminders, digital paperwork and other patient-facing workflows to existing practice data, allowing the underlying clinical software to remain in place while new capabilities were built around it.
That experience established a pattern that would continue through his later work: understanding an operating problem first, then using software and structured data to make the underlying process more visible and repeatable.
Beginning in 2020, Vitenas applied that approach to real estate investing. Over the following five years, he participated on the buy or sell side of approximately 35 Texas real estate transactions, including distressed land, subdivision opportunities, assemblages and other complex acquisitions.
His investment process increasingly incorporated software and data analysis. He built tools to identify foreclosure opportunities, used SQL to narrow large datasets into actionable prospects and developed Deal Studio to organize the research, diligence and execution involved in transactions where incomplete information and limited time could materially affect an outcome.
In 2025, Vitenas joined Upperline Companies as Vice President of Acquisitions & Operations. His work there expanded into commercial real estate acquisitions, underwriting, investor workflows and internal systems. He also developed software intended to translate parts of the firm's existing underwriting and investment processes into structured, persistent workflows.
Across those chapters, the industries changed, but the underlying problem remained familiar: important knowledge was often scattered across databases, spreadsheets, documents, inboxes and individual memory.
Vitenas OS brings that experience back to healthcare.
The project is being developed as a modern operating environment for specialty medical practices, with an emphasis on preserving the patient journey and the institutional knowledge accumulated over years of operating a practice. Rather than beginning with the assumption that existing systems and workflows should simply be replaced, Vitenas OS is being built around understanding what already works, what information matters and where better software can improve the experience.
Vitenas recently began documenting the ideas behind this approach on AlexanderVitenas.com, a personal archive covering his work across healthcare, software, investing and systems development. His writing explores recurring problems encountered across those fields, including how software can preserve context, make decisions reviewable and prevent organizations from repeatedly reconstructing their own history.
